Organ Donation Consent
The Organ Procurement and Transplantation Network (OPTN) Policies state that, as a donor, you must affirm that:
- You want to donate.
- No one forced you to donate.
- No one said they would give you something of value for donating.
- You know that you can decide not to donate at any time.

By agreeing, you consent that you understand the above restrictions and the Organ Procurement and Transplantation Network (OPTN) Policies, and that, violations of these policies are punishable by laws.
Gift O’ Life User Agreement
Gift O’ Life is designed to increase living donation of organs by providing a safe medium for organ recipients and donors to find each other. While we prohibit bribery and coercion, we are not responsible for any incidence between members.
We strongly prohibit the sharing of personal information such as, but not limited to Social Security Number (SSN), financial Information, location address.
Gift O’ Life will never ask for members’ SSN, DOB, address or financial information. The information needed to create an account are:
Name (First and Last) – Only first name is displayable to other members.
- Blood type
- Age
- Organ type
- Zip code
- Member type
Sharing of information other than listed above will result in the member’s account removal.
Members of Gift O’ Life are considered patients therefore, their information is protected and governed by HIPAA. To learn more about HIPAA, please click the link below:
Gift O’ Life may report violations to the Organ Procurement and Transplantation Network (OPTN). By agreeing, you consent to Gift O’ Life’s terms and policies of use.
By agreeing to GiftO’Life terms and conditions, members understand that they are potential recipients and donors, and as such, initial matches on GiftO’Life are uniquely based on organ types and blood types, therefore, don’t guarantee donations as other criteria may be required to fully qualify as a donor.
